if all speech is free to speak under the constitution of America. And FEMALE GENITAL MUTANTLATION advocated for in public with it being illegal could this be considered advocating abuse of a child and punishable by law?

Now a doctor can based on his moral beliefs refuse to do FGM. under the US constitution under the law.

This speech is protected under religious freedom. Not freedom of speech under the constitution. The law is not moral. (Cannot does not have to be moral) it takes people with moral values to change unmoral laws.
