The answer is much simpler than democrats vs Republican.
It is that cities with close access to hospitals etc and less hazardous work conditions are more likely to be democrats.
While rural areas have lower access medical and other support as having higher risk occupation and are more likely to vote Republican.

The old correlation is not necessarily causation…
